For all appointment types:
Any inconsistency in the information provided by the requesting party with the records of Records Office will be interpreted as a misrepresentation and the online request may be invalidated and denied regardless of compliance with all requirements.
A request to expedite is highly discouraged due to limited manpower to produce documents in school.
All personal information collected in the Student Document Request Form will be kept confidential and will only be used for its intended purpose.
All requests for soft copies are not recommended since these can be falsified.
Only the owner of the record is allowed to request for the document relative to his/her school records and the claim for the said document.
Identification of the requesting/claiming party shall be done by presenting one (1) valid ID.
Authorization letter with one (1) valid ID shall be presented by any representative/proxy to claim the document.
